Follow the following instructions if you are one of the following:

  • In-state pharmacy
  • Institutional pharmacy
  • Non-resident pharmacy
  • In-state pharmacy services
  • Out-of-state pharmacy services
  1. Using the link on the ALBOP homepage, create a Gateway profile if you have not already done so. Note this Gateway profile is a personal profile, not a profile for the pharmacy itself. For more instructions on how to do this, please view our “How to Create a New Gateway Profile” video tutorial.
  2. Log in to your Gateway, click the Applications tab, and select the appropriate new application. You will have 7 days to submit payment and 30 days to upload any required documents.
  3. Once you receive your approval email, log back in to your Gateway profile to print your permit.

Pharmacies renew their permit on even-numbered years. The deadline to renew your pharmacy permit on time is December 31 of even-numbered years. Between January 1 and 31 of the following year, your renewal will be considered late, but you can still renew online. After January 31, the online renewal window will close, and you will need a reinstatement application.

Reinstatement & Change of Ownership

Please note all reinstatement/change of ownership forms will be moving to ONLINE ONLY beginning October 1st.

To submit a change of ownership:

  1. Using the link on the ALBOP homepage, create a Gateway profile if you have not already done so. For more instructions on how to do this, please view our “How to Create a New Gateway Profile” video tutorial.
  2. Log in to your Gateway, click the green Pharmacy tile, and select Manage Owners on the lefthand side. You will have 7 days to submit payment and 30 days to upload any required documents.
  3. Once you receive your approval email, log back in to your Gateway profile to print your permit.

Managing your Permit in Gateway

Once logged in to the Gateway, you will be able to do the following:

  • Update contact info
  • Update employees
  • Add/remove yourself as supervising pharmacist (can only be done by pharmacists, not a user with admin rights)
  • Print your permit
  • Pay fee/fine
  • Add/change operations (compounding, central fill, drug collector, non-pharmacy key holder, nuclear, parenteral, remote order entry)
